La fotografía es hoy en día un elemento auxiliar indispensable en la odontología actual. La palabra fotografía deriva de las palabras griegas phos (“luz”) y graphis (“pluma” “pincel”) o graphê (“representación a través de líneas” o “dibujar”), que significa “dibujar con luz”.
La fotografía tiene un valor legal muy importante ya que permite realizar una documentación adecuada de cada paciente y de cada caso. Gracias a ella se puede realizar un diagnóstico adecuado y planificar de forma individualizada y correcta el tratamiento. Finalmente, ayuda a los observar la evolución de cada tratamiento respeto el inicio, de modo que da una garantía de tratamiento.
La fotografía llamada clínica es la que se realiza en las clínicas dentales. Esta juega un rol de gran importancia para realizar los tratamientos odontológicos.
LA FOTOGRAFÍA EN ODONTOLOGÍA
Existen varios tipos de fotografías que se utilizan en la odontología y es de importancia conocerlos ya que casa uno de ellos cuenta con una serie de características específicas que son imprescindibles para el diagnóstico y el tratamiento.
Las fotografías clínicas se diferencian principalmente en dos: extraorales e intraorales.
Las fotografías clínicas extraorales son las tomas fotográficas de la cara completa, de frente (en reposo y de sonrisa), de perfil del paciente (en reposo y de sonrisa) y la de sonrisa.
- La fotografía extraoral frontal. El paciente debe estar con la línea bipupilar paralela al plano horizontal, al suelo. La línea media tiene que centrarse y encuadrar la fotografía. La cámara tiene que ser posicionada en posición vertical, y el punto de enfoque se debe situar en la intersección del plano de Frankfurt con la línea media facial.
- La fotografía extraoral de perfil. El paciente se encuentra sentado en posición semierguida y la cabeza rotada a 45º, de esta forma no existe dependencia respecto a la forma y la posición de la nariz. El punto de enfoque se debe situar a en la intersección del plano de Frankfurt con la línea vertical que pasa por el canto externo del ojo.
- La fotografía extraoral de sonrisa. El paciente debe estar de frente, mostrando una sonrisa espontánea. Los márgenes de la fotografía deben incluir ambas comisuras labiales, la base de la nariz y el surco mentolabial. El punto de enfoque debe estar en la intersección de la línea media facial con la línea intercomisural.
Las fotografías clínicas intraorales son las tomas fotográficas que se realizan de dentro de la boca del paciente, para captar los tejidos blandos y duros de forma más detallada y las relaciones que entre ellos se establecen. Las fotografías intraorales se clasifican en tres: frontal, laterales y oclusales.
- La fotografía intraoral frontal. El paciente para realizar esta fotografía se encuentra sentado, con el plano oclusal paralelo al suelo. El visor debe estar entre los incisivos superiores e inferiores, a nivel de la línea media.
- Las fotografías intraorales laterales. Los márgenes de esta deben quedar entre los incisivos y los primeros molares, y el punto de enfoque debe estar a nivel de premolares. Se puede usar un espejo o hacerlas con visión directa.
- Las fotografías intraorales oclusales. En ellas se observa las arcadas superior e inferior completas. Se realizan mediante la ayuda espejos que se colocan hasta el último molar presente en boca. ?El visor de la cámara debe estar en el punto medio entre la línea media de la arcada y la línea que une el primer o segundo premolar. El punto de enfoque también se localiza en los premolares. ?
En la odontología, se distinguen otro tipo de fotografías, estas son nombradas como fotografías complementarias. Este tipo específico de fotografían se utilizan para poder documentar cada una de las pruebas que se realizan y se necesitan para realizar una evaluación clínica del paciente, poder documentar ciertos hábitos (como la deglución atípica), entre otros. Las fotografías complementarias son de gran utilidad para poder ponerlas en el anexo de la historia clínica y para seguir una secuencia más práctica del tratamiento.
Es de gran importancia realizar fotografías preoperatorias y post operatorias, para que de este modo se consiga centrar la atención del paciente en las diferencias entre el antes y el después resultantes al tratamiento odontológico.
